Service Description: Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible with in-home supports, meals on wheels, supportive housing, dementia care and community wellness programs * referral to other community services * volunteer opportunities
Adult Day Program - Adult day program * social, recreational, and therapeutic activities in a group setting for part of the day * helps to support family caregivers who provide day-to-day care
Caregiver Education and Support Series - Services to help caregivers cope with demands, including respite through day programs, home care relief, and support groups
Community Wellness Programs - Variety of drop-in and registration-based health promotion, recreational, social and meal programs to help prevent illness and functional decline, increase engagement in the community and improve quality of life
Counselling and Support - One-on-one supports for coordinating services, including personal advocacy * interventions for people facing homelessness, critical life changes, abuse, or isolation * referrals to other community support services as necessary
Foot Care - Basic and advanced foot care provided at community clinics * foot care includes care of corns, calluses, fungal nails, and filing and trimming of nails
George Barker Manor Supportive Housing - Supportive housing with 24 hour personal care assistance
In-Home Services - Range of home care services including homemaking, personal care and escorting
Janet Magee Manor Supportive Housing - Supportive housing with 24 hour personal care assistance
Meals on Wheels - Hot or chilled meals delivered around noon* special diet types available * includes soup, entree, bread and dessert
Montgomery Place Supportive Housing - Supportive housing with 24 hour personal care assistance
Moore Place Supportive Housing - Supportive housing with 24 hour personal care assistance
Transportation - Non-emergency transportation to local and long-distance medical appointments, and to do regular activities such as grocery shopping and banking
